The latest Academy of Breastfeeding Medicine (ABM) Protocol #36 shifts the focus away from aggressive pumping and massage, emphasizing gentle care, inflammation management, and overall breast health.
Clogged Ducts: What to Do
✔ Avoid aggressive massage – Deep tissue massage can cause more inflammation and worsen symptoms. Instead, use gentle lymphatic drainage (light strokes towards the armpit).
✔ Use heat sparingly – Warmth before nursing or pumping can help with milk flow, but prolonged heat may increase swelling.
✔ Cold therapy is key – Applying cold packs after feeding or pumping reduces inflammation and relieves pain.
✔ Keep milk flowing naturally – Breastfeed or pump as needed, but avoid excessive pumping, which can lead to oversupply and worsen symptoms.

Mastitis is now understood as an inflammatory condition, not just an infection. The new recommendations include:
