Click the online portal button to fill out, review, and sign the consent form on a secure patient records platform. Use “Boise Lactation” when it asks for practice name and please make sure and sign the “Consent to Treat” in the documents portion. Then move on to Step 2!
2. Check your Insurance Coverage
Your insurance may help cover the cost of lactation services – don’t miss out! You do still have to apply for most insurances-so see links below. If you’re not covered with Lactation Network or have out-of-network insurance, I can give you a super-bill that you provide to your insurance company for hopeful reimbursement. This option requires you to pay out of pocket for your visits. After finishing this section, please move onto the final step to schedule-See link in next section.
